FORMS, FORMS, MANDATORY FORMS

 Every student in grades 1-8 is required to submit certain forms to school.

Please check the list below to see what forms your child needs.

All forms must be completed IN INK, signed and dated.

Send all completed forms to the classroom or homeroom teacher via backpack ASAP.

All the forms are attached at the bottom of this page. If you can't open a form, TRY ANOTHER BROWSER!

KINDERGARTEN

• You’re finished. You filled yours out at registration (remember all those forms?!)

 1^ST, 2^ND, 3^RD AND 4^TH GRADES:

• 2012-13 Consent to Photograph, Film or Videotape (parents of new students already filled this out at registration.)

 5^TH GRADE:

• 2012-13 Consent to Photograph, Film or Videotape
• 2012-13 Locker Contract
• 2012-13 Young Adult Literature Access Permission

 6^TH GRADE

• 2012-13 Consent to Photograph, Film or Videotape (parents of new students already filled this out at registration.)
• 2012-13 Locker Contract 
• 2012-13 Young Adult Literature Access Permission
• 2012-13 Extended Day Permission Slip
• (6^th grade off-campus lunch doesn’t begin until January - form to come)

 7^TH GRADE:
