Mandatory Forms

Every student is required to submit certain forms to school. All forms must be completed IN INK, signed and dated. Only the School Lunch Form can be completed online!

Check the list below to see which forms your child needs to submit and scroll to the bottom of the page where the forms are attached. If you can't open a form, try another browser! Our website is Google, which is sometimes incompatible with Internet Explorer.

Send in all completed paper forms on the first day of school Wednesday, September 5, 2018 via backpack to the classroom or homeroom teacher. (Only the School Lunch Form can be completed online.)
